Insurance
Please
feel free to discuss medical costs with our office billing staff. It
is the policy of St. Mark's Rehabilitation to submit claims to insurance
carriers on behalf of our patients. Please keep our bookkeeping
department informed of any changes in your insurance to help avoid
errors. Patients are responsible for co-payments at the time of
service.
Below
is a list of insurance programs in which we currently participate.
We continue to make application as new options are offered to
patients. Please inquire if you do not find your program listed, as
our participation status continues to grow. You should be aware of
subtle benefit differences between products offered by the same insurance
carrier and should make certain that the level of coverage for your care
will be what you anticipate.

Appointments &
Consultations
To
schedule an appointment, please call 212-529-5966. Same
day or next day appointment requests are given, based upon availability. New
patients are always welcome.

Office Hours
General office hours are Monday through Thursday 8 am to 7 pm
and Friday 9am - 5 pm. Physical therapy hours are staggered from early
mornings to late evenings for your convenience. If a problem or
question arises after hours, do not hesitate to call. Our
answering service will contact the doctor to return your call or you
can leave a voicemail message.
NOTE: 24 hour notice is required for canceling
physical therapy appointments. Less than 24 hour
notice will result in a cancellation fee.

Cancellations
If
you are unable to keep an appointment, we ask that you kindly provide us
with at least 48 hours notice. This courtesy on your part will make
it possible to give your appointment to another patient.
